CHOOSE YOUR DOCTOR OR PEDIATRICIAN
Basic medical assistance is guaranteed by affiliated doctors for general medicine and pediatrics (General Practitioner – GP and Free Choice Pediatrician – PLS).
Since this is a relationship based on trust, people can exercise the right to choose a doctor among those affiliated with the Regional Health System of their municipal area, provided that they have not exceeded the maximum number of choices, barring particular exceptions.
To allow citizens to make their choice, the ASL makes the list of general practitioners and paediatricians operating in the District available at the CUP offices.
The same list can also be consulted on the Lazio Region website.
The citizen can change doctor by making a new choice by contacting the CUP offices of the District of residence. If the doctor does not intend to provide his services in favor of a patient, he can refuse it, justifying the revocation in writing.
HOW TO REGISTER WITH THE SSN FOR THE FIRST TIME
The following documents are required:
• Valid identification document;
• Self-certification of residence;
• Self-certification of Family Status for families;
• Self-certification tax code of each family member;
• SSN registration document of the ASL of origin;
• Proxy signed by each adult member of the family unit (husband/wife; parent/adult child).

HOW TO REGISTER NEWBORNS
The following documents are required:
• Identification document of the parent or person exercising parental authority;
• Self-certification of family status;
• tax code of the newborn issued by the Revenue Agency;
• Declaration that the other parent, holder of parental responsibility, agrees with the choice in the event that the request is presented by only one parent (Declaration in lieu of notoriety form);
Persons other than parents or those exercising parental authority must be provided with a proxy together with the identification document of the delegate and the delegating party.

The following documents are required:
• Valid identification document;
• SSN registration document.
• For minors, also a declaration that the other parent, holder of parental authority, agrees with the choice in the event that the request is presented by only one parent (Declaration in lieu of notoriety form);
If it is not possible to go to the ASL counters, the application can be presented by a trusted person with a signed proxy and necessary documentation, as well as a document identifying the delegate and the delegating party (“proxy form”).

It is possible to choose or change a General Practitioner even from home through the Lazio Region portal, via a guided registration.

HOW TO TEMPORARILY APPOINT YOUR DOCTOR OR PEDIATRICIAN
Citizens temporarily domiciled and not resident in the ASL have the right to register with the SSN and therefore have the right to medical assistance provided by a General Practitioner (GP) and, for children aged from 0 to 6 years, to that provided by a Free Choice Pediatrician (PLS).
The temporary appointment of a General Practitioner (GP) or Free-Choice Pediatrician (PLS) must have a duration of between three months and one year, possibly renewable, and it is possible to request it if you belong to one of these categories:
• military, religious, hospitalized in healthcare institutions;
• citizens who request provisional registration for work reasons and dependent family members;
• citizens who request provisional registration for study reasons;
• seasonal workers for a period exceeding three months;
• patients temporarily domiciled for health reasons (certified by the attending doctor);
• teachers with assignments lasting more than three months;
• posted workers (and their family members) for a period exceeding three months;
• career soldiers and their families;
• public and private employees (and their family members) on assignment in a location other than their place of residence for more than three months;
• prisoners (under house arrest);
• elderly people (> 65 years) or disabled people who need assistance from their family members in a municipality other than their place of residence.
To choose the temporary General Practitioner, the procedure is as follows:
• communicate the request for revocation of the General Practitioner to your ASL of residence in the manner established by the competent region;
• send an email to scuolavocamedico.tivoli@aslroma5.it, or go to the office of the District of temporary domicile, delivering a copy of the valid identification document, the Tax Code and the self-certification of domicile, self-certification certifying the eligible category and form of request;
• for the elderly or disabled, also a declaration in lieu of an affidavit of the need for assistance from family members in a municipality other than that of residence.
It is recommended to provide a telephone number for any clarifications and/or communications.
Notification of the completion of the case will be given via email.

HOW TO REGISTER IF YOU ARE A NON-EU WORKER WAITING FOR EMERGENCE
Non-EU workers awaiting emergence have the right to register with the SSN and therefore have the right to medical assistance provided by a General Practitioner (GP).
Temporary registration lasts six months, but can be renewed until the citizen is summoned by the competent authorities to settle their case.
To choose the temporary General Practitioner, it is necessary to send an email to scuolavocamedico.tivoli@aslroma5.it of the relevant District attaching the following documentation:
• The Tax Code, which will be numerical until regularization occurs, issued by the Revenue Office;
• Identity document (passport);
• Copy of the document proving the request for regularization or emergence from irregular work;
• The employment contract.
